How Capilclinic doubled its presence in AI answers
Ask ChatGPT or Gemini which hair transplant clinic offers the best value in Spain this year. The answer comes back as a shortlist of three or four names. Every clinic outside that shortlist is invisible, whatever its website ranks for. Capilclinic wanted to be on the list.
The starting point was measured, not assumed
Before publishing anything, we mapped where the brand actually stood. Ninety-seven prompts, three topics (clinica capilar, trasplante capilar, resultados trasplante capilar), Spanish market, measured with Qwairy and Meteoria. Capilclinic surfaced in roughly a third of AI-generated answers: 35% on clinica capilar, 39% on trasplante capilar, 31% on resultados trasplante capilar.
Two things stood out. The brand was already present enough to be reachable by the models, and competitors were close enough that the topic was winnable. That combination is what makes a category worth attacking off-site.
We read the models before writing for them
The instinct is to publish where the brand wants to be seen. The method is to publish where the models already look. We identified the media that LLMs actually pull from when they answer Spanish-language questions on hair transplant topics, then built the plan around those specific outlets.
Eleven articles followed, written in AI-First™ format and mapped to the real questions users type, structured so an answer engine can lift them as a source rather than merely crawl them.
The performance score doubled on two of three topics
Between October 2025 and January 2026, visibility moved on every topic studied. Clinica capilar went from 35% to 71% (+103%). Resultados trasplante capilar went from 31% to 63% (+103%). Trasplante capilar went from 39% to 58% (+49%).
The competitive picture moved further. Capilclinic went from 34 mentions to 69, a gain of 35 mentions in three months. Hospital Capilar added 10 (21 to 31), Clínica Esbeltia added 16 (13 to 29), Castellana Clinic added 14 (13 to 27). The gap between Capilclinic and its closest competitor widened from 13 mentions to 38. Share of voice is a volume game, and volume is exactly what off-site publication buys.
Nine articles out of eleven produced a mention
The conversion rate is the number that matters operationally. Out of 11 published articles, 9 directly enabled the brand to appear in an AI-generated answer. That is 82% of the media budget converting into visibility inside the answer itself.
Two examples show the mechanism at prompt level. On "¿Qué clínicas ofrecen la mejor relación calidad-precio en tratamientos de injerto capilar este año?", Capilclinic was absent from the answer. After publication in La Razón on 18 November 2025, the article is cited as a source and the brand appears in the response on page one. Same pattern on "¿Top 5 clínicas capilares en 2025 según la satisfacción del cliente?": absent before, then cited and mentioned after publication in Madridiario in January 2026.
The article becomes the citation. The citation becomes the mention. The mention becomes the recommendation the user reads.
What this case actually proves
AI visibility is decided outside your own domain. The models read the press, the comparison pieces, the specialist media, and they rebuild an answer from what those sources say. Owning your website gives you no vote in that conversation.
Monitoring tools tell you where you stand in AI answers. Publication changes it. The distance between the two is the whole job.
